Legal Release and Waiver
In connection with my voluntary involvement in activities undertaken for and with the participation and support of Project Live Love, a non-profit charitable organization, I hereby agree for myself, my heirs, assigns, executors and administrators to forever release, discharge and covenant not to sue Project Live Love, its officers, directors, employees, agents, volunteers and assigns, for any liability, claims, demands or actions for injuries or losses sustained to my person and/or property as a result, directly or indirectly, of my involvement in such activities, whether or not resulting from negligence of Project Live Love, its officers, directors, employees, agents, volunteers and assigns. I hereby attest that my attendance and involvement in such activities is voluntary and that I am participating at my own risk. Furthermore, I grant and convey unto Project Live Love, all right, title and interest in any photographs, video or audio recordings taken of me, or quotations from me during my volunteer activities, which may be used by Project Live Love for promotional purposes in any media.
I hereby confirm, represent and warrant that I have never been convicted of, or charged with, a violent crime, child abuse or neglect, child pornography, child abduction, kidnapping, rape, or any sexual offense whatsoever, and I have read (or have had read to me) this Volunteer Release & Waver of Liability and understand the legal effects of its contents.
